1. Download Covid-19 Healthcare workers EXCEL template.
2. Fill it in CAPITAL letters only.
3. Please do not use copy/paste and any type of modification on existing data of this template.
4. Fill the template with dropdown options and typing.
5. Name of XL sheet not to be tempered. Just add your facility name by putting Underscore( _ ) at the end like
   COVID VACC_IMPORT BENEFICIARIES_STATES_UT_DEHRADUN_MAXHOSPITAL
6. After filling the data it is compulsory to send the certificate .
